Certificate of Identity

We issue temporary travel documents called Certificate of Identity (CI) for infants born in Thailand to Maldivian parent(s) and for Maldivians in Thailand whose passport is either lost, stolen or destroyed.

 

CI for an infant

The following documents are required to issue a CI for an infant, The usual processing time for the CI is about 4 working days.

  • Passport size photo (white background)
  • Birth document issued by the hospital where the infant was born
  • Marriage Certificate of the parents
  • Confirmed air ticket
  • Certificate of Identity (New Born) application form filled by a parent
  • Completed passport application information form for Maldivians abroad (IM 32)

 

CI for a Maldivian whose passport is either lost, stolen or destroyed

The following documents are required to issue a CI for a Maldivian whose passport is either lost, stolen or destroyed. The usual processing time for the CI is about 4 working days.

  • A report from your nearest Thai police station stating that the passport is lost, stolen or destroyed.
  • An English translation of the police report (a translation can be obtained by visiting a translation centre at the rate of approximately 1000 Baht per page). Please contact the Embassy if you need assistance with finding a translation centre.
  • Duly filled Certificate of Identity application form
  • Duly filled Passport application information form for Maldivians abroad (IM 32)
  • Data page of the Passport (if available)
  • 2 Passport size photos (white background)
  • Confirmed air ticket
